I believe moon patrol skips because it refreshes at 57 Hz. When you turn on vsync you will still get small skips because your monitor is probably not running at the same refresh rate. Syncrefresh will fix the skipping problem but will speed up the emulation to match your refresh rate(check with F11) and your sounds will also probably start skipping. Triple buffer should work, but I don't know because my G400 doesn't support it.
