Multi-Mouse Mame is at http://www.urebelscum.speedhost.com. It supports two players, the winMAME version is USB and the DOS Version is PS/2 (???) and serial (definitely), but doesn't save config settings.

There is also an old (R37b12?) port of EMU+ which is PS/2-serial DOS and does save config settings. It is linked from www.arcadeathome.com
